MINOR CHILDREN JOINING LAWSUIT FORMS ARE NEEDED
We prefer to type your form and mail it to you to sign, but if you want to try to fill it out yourself or need to re-do a form we sent you, please follow these directions below.
Please follow the instructions carefully to fill the new form. This form is called a Guardian Ad Litem form (GAL) and it is used to give representation to a minor child by having an adult sign the form agreeing to be the child’s legal representative with the attorney.
There are two signature areas on GAL form for Clients to sign on the second page of court form, below where the attorney will sign next to his printed name. Blue Ink Only.
The first signature area is called “Applicant”, which is either YOU, the parent or guardian, OR your older minor is considered the Applicant if age 14 to 17. If your child is under 14, then you will be the Applicant and print and sign your name here.
Below that, in the last signature space, YOU, the parent or guardian, will print your name and sign. You may type this form on our website and then print to sign signatures.
PLEASE FILL FORM THEN PRINT FORM TO SIGN AND SCAN BOTH:
(1.) MINOR’S “GAL” FORM and
(2.) MINOR’S RETAINER FORM BACK TO US. (one per child)
We will review and then give you the Green Light to mail in originals.
Remember, only the Adult will sign the Retainer Form for the minor child. Minor’s name and age on page one and Adult information and signature on page 2- for ALL minors under the age of 18.
